Certificate Requests

• Parents can only request sacramental certificates for their children under the age of 18. Once over the age of 18, they are Adults and permission must received by the person him/herself.  The same applies for picking up the certificate.
• Godparents, siblings, in-laws or friends cannot request sacramental certificates. The same applies for picking up the certificate.
• Priests, Women Religious & Deacons can request sacramental certificates. The requests, must be made in writing.
• Sacramental records can NEVER be faxed or scanned and emailed​

Certificate may ONLY be requested by phone or in person.

Please allow up to 3-5 business days for certificate(s) to be ready, once it is ready the office will contact the phone number that has been provided. Once you have submitted your form please contact the office and notify us.
If the sacramental certificate is to be mailed, donation must be mailed first (personal check, money order or cashiers check).
​Once the donation is received, the certificate will be mailed to the address requested.
​There is a $10.00 donation per certificate printed or mailed.
